New Patient Policies and
Website Privacy Statement

PAYMENT AGREEMENT
Health, dental and accident insurance policies are an arrangement between an insurance carrier and a patient. Allcare will prepare or assist patients in preparing any necessary reports and forms to assist the patient in collecting payment from their insurance company and that any amount authorized to be paid directly to this office will be credited to the patient’s account upon receipt. All services rendered to a patient are charged directly to that patient and the responsibility of payment rests with that patient. Allcare, as a courtesy to its patients, will not impose any interest charges to any balance a patient may incur for a period of thirty days after the charges are incurred. This courtesy will not impose any interest charges to any balance a patient may incur for a period of thirty days after the charges are incurred, to afford the patient’s insurance carrier, if any, that amount of time to make payment. Any balances outstanding for more than thirty days will be charged interest at the rate of 1.33 percent per month or the maximum rate permitted by applicable law. In the event a patient’s account balance is referred to an agency or attorney for collection purposes, the patient agrees to pay reasonable attorney fees and any expenses or costs relating to the collection proceeding, including court costs. In the event that the patient is a minor, the parent and/or guardian of said patient is responsible for all services rendered to the patient herein. If a patient suspends or terminates care and treatment, any fees for professional services rendered will be immediately due and payable. (See Interrupted Services Policy below)


DENTURE PACKAGES – (this section applies only to patients receiving Immediate Denture or Immediate Partial packages)
Denture work being provided to a patient is priced as a complete package and that in the event that a patient fails to return to Allcare to receive his or hers Permanent Denture within the latter of: (a) twelve (12) months from the date the immediate denture is received; or (b) eight (8) months from the last date of care from Allcare with respect to denture treatment plan, the patient will not be entitled to any refund or credit to his or hers account. However, in the event that the patient informs  Allcare in writing of his or hers decision not to accept the Permanent Denture prior to the earlier of Allcare manufacturing the Permanent Denture or twelve (12) months from the date that the immediate/temporary denture was delivered, the patient will be eligible for a $200 refund for the Permanent Denture.


REFUND POLICY
Should a patient request a refund from Allcare, the request should be made to the Manager of the Allcare facility where he or she is a current patient. The Manager will forward a Refund Request to a Supervisor for final approval. During this process, the patient’s account will be reviewed and any fees for professional services rendered, including interrupted services fees if applicable, will be immediately due and payable. Refunds will be administered in the same manner in which the initial payment was made to Allcare. Further, the refund process can take up to fifteen days to finalize and up to 30 days for a third party lender, if applicable, to reflect the refund to an account.


BOUNCED CHECK
Allcare reserves the right to charge a $25.00 fee for bounced checks.


SENIOR DISCOUNT
Senior Discounts are for patients 65 years or older.  Terms of this discount vary.  This discount may not be combined with certain other discounts. (See office for details)


COUPON DISCOUNTS
Coupon Discounts are for patients receiving a certain type of treatment.  Terms of this discount may vary.  This discount may not be combined with certain other discounts.  (See office for details)


INTERRUPTED SERVICES POLICY
Some treatment may take more than one visit to complete.  In the event that a patient cancels treatment before all visits are completed according to the treatment plan, Allcare reserves the right to charge the patient for work rendered up to the point of cancellation.


MISSED APPOINTMENT POLICY
In the event that a current Allcare patient (excluding new patients) misses an appointment or fails to inform Allcare that he or she will miss an appointment within 24 hours of the scheduled appointment, Allcare reserves the right to charge the patient a $75.00 missed appointment fee.

Allcare does not charge new patients for missed appointments.


UNSCHEDULED TREATMENT POLICY
It is a goal of Allcare to provide our patients timely servicefor all patients.  Often times patient’s scheduling conflicts prevent them from returning to complete their entire treatment. In these situations it is imperative that contact is made with the patient to reschedule treatment before treatment changes or minor dental related issues become more serious.  Allcare reserves the right to charge an administrative fee to pre pay patients for its attempts to contact the patient to continue treatment.  Allcare shall have the right to charge a reasonable administrative fee of $10.00 per phone call, and $20.00 per letter sent, in an effort to schedule the completion of his or hers treatment.  Phone calls and Letters are completed as follows: Month 1- phone call, Month 2- phone call, Month 3- letter, Month 4- phone call, Month 5- phone call, Month 6- letter, and so on.
